	Diagnostics
	Glaucoma
	Steroids
	Steroid Combinations
	OTC Medications

When ordering pharmaceuticals, state law requires that a copy of the doctor's professional license and TPA/DPA certificate (optometrists) be on file in our records. Mail or fax copies of the appropriate license/certificates. [This does not need to be repeated on subsequent orders unless the status of the professional license should change (i. e. a move to another state)]
